Reablement Support Coordinator
Hospital Discharge & Reablement Team
Permanent, full time (37 hours per week)
Salary £21,589 - £23,836 (Grade 5)

Who you are…
- Qualified to NVQ Level 3
- Experience of leading and supporting a small staff team
- Have a positive attitude to disability and an understanding of the purpose of advocacy
- Experience of Person Centred Planning and Health Action Plans
- Good verbal and written communication skills
- Able to prioritise own and others work, and motivate staff members
- Able to work flexibly as part of a rota to include weekends, bank holidays and evenings.
