Reablement Support Coordinator

Hospital Discharge & Reablement Team

Permanent, full time (37 hours per week)

Salary £21,589 - £23,836 (Grade 5)

Who you are…

  • Qualified to NVQ Level 3
  • Experience of leading and supporting a small staff team
  • Have a positive attitude to disability and an understanding of the purpose of advocacy
  • Experience of Person Centred Planning and Health Action Plans
  • Good verbal and written communication skills
  • Able to prioritise own and others work, and motivate staff members
  • Able to work flexibly as part of a rota to include weekends, bank holidays and evenings.
